CREDIT

Physicians: 

This activity has been planned and implemented in accordance with the accreditation requirements and policies of the Accreditation Council for Continuing Medical Education through the Joint Providership of the West Palm Beach VA Medical Center and Palm Beach County Medical Society. The West Palm Beach VA Medical Center is accredited by the Florida Medical Association to provide medical education for physicians. 

The West Palm Beach VA Medical Center designates this education activity for a maximum of 4 hours (per session) AMA PRA Category 1 CreditTM. Physicians should only claim credit commensurate with the extent of their participation in the activity.  

DISCLOSURE  

It is the policy of the West Palm Beach VA Medical Center to ensure balance, independence, objectivity and scientific rigor in its educational activities. All prospective faculty and planners of CME activities are required to disclose, in advance of participation, any financial relationships with commercial interests related to the subject matter of the activity. The disclosures are reviewed for conflicts of interest (COI) by the CME office and resolved through an established COI policy prior to the presentation. Relevant financial relationships are disclosed to participants so that they may formulate their own judgments regarding the presentation. 

Faculty are required to inform the audience when they are discussing off-label and/or investigational use of pharmaceuticals or instruments in their presentation.
